Apply

To attend the Survivor Conference, you must:

Step 1 – Complete a Survivor Conference application form (download as a PDF or a Word document ). If you are bringing a supporter, complete the Supporter application form (download as a PDF or a Word document). Once you’ve completed the form, send it to us either by e-mail to (JavaScript must be enabled to view this email address), by fax to (709) 579-7326 or by mail (Young Adult Cancer Canada, P.O. Box 8171, St. John’s, NL, A1B 3M9).

Step 2 – Once we receive your application, we will review it. Once it has been reviewed, we will contact you and inform you of your eligibility to attend. We promise we’ll be quick.
